Become an InfantSEE Provider

InfantSEE is an opportunity to educate families in your community about the importance of annual comprehensive eye and vision care beginning at six months of age and annually thereafter.
During an InfantSEE assessment doctors of optometry assess many areas such as testing for excessive or unequal amounts of nearsightedness, farsightedness, astigmatism, eye movement ability, eye health problems, and discuss patient and family history.
Need more information about your role as an InfantSEE provider? Head to our InfantSEE OD Center for more information.
Ready to become an InfantSEE provider? Please fill out Become an InfantSEE Provider form, and we will send a welcome e-mail with more information when we enroll you as a provider.
Thank you for helping ensure a lifetime of healthy vision for our smallest patients!
